REQUIREMENTS FOR RELEASE FROM SOCIAL SERVICE:
4 ID size photographs.
Not snapshots. Recent. No shine. B/W front photos. Badge size 3.5 x 4.5 without frame. Formal wear, men and women jacket, plain color, men tie. Full name on the back of each photo.
Original proof of payment, signed and stamped by the banking institution.
Write down on the payment receipt: Name, code, career, landline and email.
The unit is not responsible for lost váucher that does not have the data of the interested party written down.
Likewise for payments made to a different bank reference than the corresponding form indicates.
1 Copy of the identification sheet.
1 Copy of proof of affiliation and acceptance.
1 Copy of Social Service initiation letter.
Original termination letter.
Change of ownership document. (if applicable)
Copy of the Kardex.
Original Global activity report. (with stamps and signature of the head of the agency)
